YOUNG PERSON'S LINK WORKERS

Our flexible, community-based link worker service connects 11-18 yr olds with local support & activities to improve wellbeing

Culm Valley Primary Care Network (PCN) has partnered with Space CC and Young Devon to set up a new Youth Link Worker Service working across our 5 member practices.


If you are aged 11-18 and registered at any of the following surgeries, you can ask your GP to be connected with a link worker.


Their role is to spend time 1-1 finding out what matters to you, and to connect you with support and opportunities in the local area. 

Young people who have worked with our link workers have told us they value the space and time to discuss things that are important to them, and that working in this way helps improve confidence, resilience and wellbeing.


This service is available to anyone registered at the following practices in the Culm Valley PCN:

  • College Surgery Partnership

  • Sampford Peverell Surgery

  • Bramblehaies Surgery

  • Wyndham House Surgery

  • The Blackdown Practice
