Culm Valley Young People's Link Workers

Culm Valley Primary Care Network (PCN) has partnered with Space and Young Devon to set up a new Youth Link Worker Service working across our 5 member practices.


Our flexible, community-based link worker work 1-1 with young people aged 11-18 yrs old, to identify strengths, and connect young people with local opportunities and wider services.

Young people who have worked with our link workers have told us they value the space and time to discuss things that are important to them, and that working in this way helps improve confidence, resilience and wellbeing.


This service is available to any young person aged 11-18 yrs, registered at the following practices in the Culm Valley Primary Care Network:

  • Sampford Peverell Surgery

  • Bramblehaies Surgery

  • Wyndham House Surgery

  • The Blackdown Practice

To access this service, ask your local GP practice about a referral.
